The earliest sign is an increase in sebaceous secretion speed, which
makes the skin appear oily (seborrhea). Typically, comedones or
blackheads appear with the oily appearance, mostly around the sides
of the nose and forehead. Comedones are follicular plugs composed of
hardened sebum and follicular waste material. In severe cases,
nodules turn into cysts with fluctuation. These lesions are actually
pseudocysts because they lack epithelial layers. This is considered
severe acne.

The most affected areas are the face, cheeks, chin, nose, forehead,
neck, back, chest, and shoulders.

The natural course of the disease is to heal on its own. However, it
is unpredictable when acne will heal. Acne typically disappears by
the age of 25. However, in some patients, it may persist much
longer. Acne tends to improve in the summer, with sunlight and
saltwater helping many patients. However, warmth does not improve
it, and truly hot and humid climates may cause acne to become
inflamed and exudative.

Acne is seen in about 70% of the population during adolescence.
However, only 10% of these people need medical assistance. It is

Histologically, the main feature is an excessively inflammatory
folliculitis. In the early stage, follicular plugs can be seen.
Later, there is an accumulation of inflammatory cells, histiocytes,
and multinucleated giant granulomatous reactions. Long-lasting
lesions have fibrous tissue deposits that initiate scar formation.
It is thought that significant inflammatory lesions in acne result
from follicular rupture.
